Agreement to these terms is required prior to matriculation to UIWSOM and valid for the duration of enrollment at UIW and/or UIWSOM. The UIWSOM Financial Agreement outlines the UIW and UIWSOM policies regarding registration, health insurance and billing.To confirm transcript status, contact Office of Admissions and Student Affairs at JComplete the UIWSOM Financial Agreement.Official transcripts with the final course grades and degree conferred can be sent to UIWSOM at or mailed to University of the Incarnate Word, Document Receiving, 4301 Broadway, CPO #304, San Antonio, Texas 78209.Transcripts need to be sent to UIWSOM directly if AACOMAS has an incomplete transcript, such as lacking degree conferral, in-progress coursework, or additional coursework is not recorded. UIWSOM accepts final, official transcripts from AACOMAS.To confirm prerequisite coursework status, contact the Office of Admissions and Student Affairs at JEnsure complete official transcripts are on file.All prerequisites must be completed by Jwith a grade of C or better. Shortly after acceptance, UIWSOM emails learners with incomplete prerequisite coursework.Complete the order form to indicate your preferred size. UIWSOM provides each learner with one white consultation coat and two sets of Cherokee brand scrubs.Matriculation to UIWSOM is contingent on the successful completion of these tasks in a timely manner.
